Insurance Information
Health Matters accepts assignment from all major insurance companies. We perform a thorough benefit analysis by calling your insurance company and verifying your benefits and then coordinating that information with the treatment protocols recommended by the doctor. We are out of network.
There will be a deductible and some sort of percentage of the total of each visit which is the patient's responsibility. We file all charges with your insurance and accept assignment. If you would like us to verify your insurance prior to coming into our office, you may download forms placed on our Website.
We follow up on all reductions, restrictions, and denials that do not fall in line with the information we initially received when your insurance was verified. The patient's portion of any deductible and/or percentage of the total visit charge are due when the service is rendered.
Supplements and Avatar sessions are often not covered by insurance and so we require the patient to pay in full for those at the time the service is rendered.
